Wisdom Tooth Removal/ Surgical Removal Of Wisdom Tooth
What is it?
Do I need it?
Wisdom tooth removal is the removal of your wisdom tooth (3rd molar) either through simple extraction or through surgical removal.
Signs/ Symptoms that implies the need for wisdom tooth removal.
- Multiple episodes of pain that occurs on and off at the back of the mouth.
- Usually, pain occurs on and off throughout a few months.
- Pain gets worst with each episode.
- Difficulty chewing at the site of problem.
- Swelling, Fever
- Difficulty sleeping at night due to pain.
What causes the wisdom tooth to become inflamed/painful?
-Not enough space for the tooth to erupt fully, leading to partial eruption of tooth. When a tooth is partially erupted, the gums overlying it will cause food traps, which leads to bacteria growth and inflammation of gums.
-Wrong direction of eruption. When a wisdom tooth grows in the wrong direction, as shown in picture below, it will have no way to erupt fully into the mouth. The wrong position causes the formation of an ideal place for food accumulation that causes inflammation of the gums and bone.
- Sometimes, impacted wisdom teeth that remain buried beneath the gums and bone can also cause pain. This is often due to their position or insufficient space in the jaw, preventing them from erupting properly.
Why should I remove wisdom tooth when it does not cause any pain?
-Some of the wisdom tooth erupt in a position that causes food trap in between itself and the neighbouring tooth, leading to the decay of the neighbouring tooth. This situation is very commonly seen in a wisdom tooth that is erupted in a mesially/horizontally tilted position as seen in picture below. The doctor will then advise you to remove your wisdom tooth to prevent decay or loss of your adjacent tooth.
Do I need to take a X-Ray for my wisdom tooth?
Yes, normally an X-Ray (OPG/ Panoramic radiograph) is taken before the doctor execute any treatment plan. The dentist will determine whether the position of your impacted tooth need to be removed via surgery or can just be removed by simple extraction. Surgical removal is commonly needed to remove lower impacted wisdom tooth.
Why do I need to remove my upper wisdom tooth along with the lower wisdom tooth removal?
Usually, the dentist will also advise you to remove the upper wisdom tooth along with the lower to prevent supraeruption/extrusion of your upper tooth due to the soon to be missing opposing wisdom tooth that is planned for removal.
How long does the procedure for surgical removal of wisdom tooth take?
The procedure usually takes around an hour.
How is the surgical removal of wisdom tooth like? What are the procedures or steps for surgical wisdom tooth removal?
Step 1: Numbing cream/gel/spray onto surgical site.
Step2: Injection of numbing medication.
Step3: Ensure you are comfortable and do not feel any pain anymore. (The pain sensation will be blocked, but you will still be able to feel movements)
Step4: The dentist will open a window through your gums and may remove some bones and cut the tooth into smaller manageable size to remove it.
Step5: Close the wound up with stitches/sutures.
Step6: Give you instructions on how to take care of your wound.
Step7: We will see you after 1 week if you have sutures placed that need to be removed.
What to expect after surgery?
You will feel numb for around 2-3 hours. Once you regain sensation you might feel some pain which can be manage usually with medications and proper hygiene care. You might experience swelling for a few days. Thus, the doctor will normally give you a 3 days MC for you to rest and recover.
How to take care of the wound after surgical removal of wisdom tooth?
After surgical removal, the doctor will give you a gauze to bite down on. You may experience some/mild bleeding at the surgical site for the first 24-48 hours. If you see some bleeding, do not worry just use the gauze and bite on it for around 20-30minutes. Refrain from using straw to drink water, rinsing harshly, hot food/drinks, spitting, on the day of surgery as it will cause the blood clot to dislodge affecting the healing of the wound.
(If you experience heavy bleeding that won’t stop even with pressure, immediately contact us or go to your nearest medical clinic/dental clinic. )
To take care of the wound, use the medicated mouthwash that is given to you after meals. Keep the mouthwash in your mouth for at least a minute before spitting it out. To allow longer contact of mouthwash in your mouth, refrain from rinsing with water after using the mouthwash. (Do take note that this mouthwash can cause staining if used for long periods more than 2 weeks. )
Keep wound area clean by gently cleaning it. Make sure you still brush and floss your other teeth.
Also remember to finish the medications (exe:antibiotics) that are given to you. You can stop taking your pain medications once you feel you are okay without it.
Contact us, if you feel that the pain at the wound site is getting worst even after 4-5 days. As it could be a sign that your wound is infected and your wound needs to be cleaned for it to heal well.
How long does it take for the wound to heal after wisdom tooth removal/surgical removal of tooth?
It usually takes around 1-2weeks for the overlying gums to heal. And around 3-6months for the bone inside to form fully.
